Care Management for Florida Seniors, Elderly, and Disabled
Professional geriatric care
managers provide a variety of services to elderly individuals and their
families. Often family members are overwhelmed and frustrated with either the
lack of information or the overload of information needed to care for their
aging loved one. Care managers can ease the feeling of helplessness by
providing the following services:

An initial
assessment is provided to identify the elderly adult's personal
and financial needs and capabilities. The ultimate goal is to enhance the client's overall level of functioning,
promote independence, and provide the least restrictive living situation
possible.

Home
health and/or homemaker/companion services can be arranged if
needed and monitored to insure quality service.

Counseling
is available to both the client and family to help them understand and
adjust to the aging process.

Placement
assistance and continued monitoring are provided should
placement in an assisted living facility or
nursing home become needed.

Crisis
management services help insure peace of mind when family
members live at a distance.

Transportation
to medical appointments and social events can be arranged to promote the
physical, mental, and emotional health of each client.

Money
management services are available as needed.

Information
and referrals to community resources, medical providers, and
legal council is available.
